Online Consent Form

I desire to participate in activities conducted by the Singapore Paddle Club (‘SPC’), including and without limitation races, camps, trainings, lessons, community activities, and beach and coastal clean-ups (together ‘Activities’, singular ‘Activity’). My participation may be in any capacity such as, a paddler, trainee, instructor, contestant, organiser or volunteer.

In consideration of allowing me to participate in any of the Activities, I accept and agree as follows. Risks and potential hazards (together, ‘Risks’) are inherent to, and may arise from, the Activities. Such Risks include or may result in personal injury (physical and/or mental) or death, and loss or damage of property. My participation in each Activity, and acceptance of these Risks (whether or not foreseeable or told to me), are entirely voluntary.

I am responsible for the security of my property. I store all property within SPC’s premises, or any area allocated to me, at my own risk.

I acknowledge that any health or medical condition, previous or current injury, and medication I am presently taking may increase the Risks to myself and others. I accept that I am responsible for my safety and well-being, and

1. Declare and confirm that I am in good health and fitness to participate in any Activity, and have not been advised against participating by a qualified medical person;

2. shall inform the person-in-charge if I feel unwell before or during an Activity; and

3. consent to receiving assistance and treatment deemed necessary in the event of an injury, accident, or illness whilst participating in an Activity, and to bear the expenses incurred.

I shall adhere to these participation rules for each Activity:

1. Comply with all reasonable instructions given to me;

2. Not act in a dangerous or reckless manner, or such manner that may endanger myself or others; and

3. Not participate in any Activity under the influence of alcohol or drugs, other than the lawful use of medication.

SPC may refuse my participation in any or all Activities if I have not complied with this agreement or, in its opinion, my participation may adversely affect the conduct of an Activity or endanger myself or others.

I will not hold SPC responsible, or be entitled to a deduction of the fees paid or payable where:

1. An Activity is suspended or cancelled due to unfavourable weather, direction or non-approval of authorities, pandemic, or circumstances beyond the reasonable control of SPC; or

2. SPC refuses my participation in any or all Activities due to any cause in paragraph 5 above.

To the extent permitted by law, SPC and its officers, members, employees, contractors, volunteers, representatives, and agents shall not be liable for any costs, losses or damages (including indirect, consequential, special and punitive damages), and claims (including third party claims) arising out of or in connection with the Activities.

I consent to:

1. the collection and processing of my personal information by SPC, and its sub-processors on a need-to-know basis, for the purposes of organising and carrying out any Activity;

2. images, photographs and videos being taken of me during any Activity; and

3. SPC reproducing, modifying, using, publishing and distributing (together ‘Use’), and transferring to others for their Use, my images, photographs, videos, quotes and feedback for the purposes of analysis, reports, audits, news reporting, publicity, and the promotion of SPC, the sport of ocean paddling or any Activity.

The laws of Singapore (without regard to any conflict of laws principles) govern this agreement, and SPC and I submit to the exclusive jurisdiction of the Courts of Singapore.
