INDIVIDUAL MEMBERSHIP: Member name = Firstname Lastname FAMILY MEMBERSHIP: Member name = Lastname family (e.g. Doe family)

Notes: -Junior (must be under 18 by end of the year) -Family Membership (all members must reside at the same address) Important note: registration is a two part process. Once you complete this form, you will be sent a confirmation email with a link. You must click the link and select the drop down menu at the bottom to complete your registration.
